► 𝗠𝗮𝘁𝗰𝗵𝗱𝗮𝘆 𝗧𝗲𝗮𝗺𝘀𝗵𝗲𝗲𝘁
This form can be used if your matchday registration book is unavailable. Please remember that for each match referee, opposition & league secretary all require a copy of this completed form

► 𝗣𝗹𝗮𝘆𝗲𝗿 𝗥𝗲𝗴𝗶𝘀𝘁𝗿𝗮𝘁𝗶𝗼𝗻 𝗙𝗼𝗿𝗺
This form is to be completed when registering a player to a team so that they are eligible to play.

► 𝗣𝗹𝗮𝘆𝗲𝗿 𝗧𝗿𝗮𝗻𝘀𝗳𝗲𝗿 𝗥𝗲𝗾𝘂𝗲𝘀𝘁 𝗙𝗼𝗿𝗺
This form is to be completed when a player registered to a BVFL team wishes to transfer to another BVFL team.
