Player Registration Forms

Could all Coaches / Managers please ensure that the correct registration form is completed. These are the ones distributed at the AGM to your league representative.

The forms are pre-printed onto A5 sheets and both sides need to be completed and returned to your leagues Age Group Representative - details of which were also handed out.
