To report what you believe to be an unsatisfactory performance by the Referee or Assistant Referee

This is the form you must use to submit your report to OBDSFL Referees Secretary. You can email a summary to the General Secretary after the game BUT only when the Report Form is received will the matter be investigated (unless the matter is consider serious by the Management Committee).
