Match Postponement Form

Standard Form attached for 2013/14
If any scheduled fixture is postponed for any reason the attached match postponement form should be completed and set to Phil Blakey Fixtures Secretary within 2 days of the date of the match, along with any other evidence, such as letters from the Ground owner etc.
